Can't afford to miss work or other appointments?

No problem!

You can easily fit us into your schedule by reserving a convenient time slot online. Same day afternoon and next day options are available. Time slots will be assigned on a first come-first served basis. If your preferred time is not available, we will try to accommodate you within 30 minutes of your time choice.

Simply fill in the online form below. We will confirm your time slot via email.

*WellFast is closed on Sunday and some holidays.
Check the WellFast Calendar on the left sidebar for upcoming closed dates.
When selecting an advanced registration time please keep in mind that Next Day means the next day that WellFast is open.

While you await the confirmation email, you may prepare the necessary forms available below and either print and bring them with you, or email them ahead to WellFast.
